Driving Directions

Northbound on I-405:

  • Take Exit 20-B [NE 124th St]. Stay left on ramp to go to NE 124th.

  • At the light, turn left onto NE 124th St.

  • Cross the bridge. At the second light, turn right onto 116th Ave NE

  • At the crest of the hill, turn left at the light onto NE 128th St.

  • At the second driveway, turn left into the parking lot at 11415 NE 128th St.

Southbound on I-405:

  • Take Exit 20 [Totem Lake/NE 124th St] and remain in the right-hand lane.

  • At the light, turn right onto NE 124th St.

  • At the next light, turn right onto 116th Ave NE.

  • At the crest of the hill, turn left at the light onto NE 128th St.

  • At the second driveway, turn left into the parking lot at 11415 NE 128th St.

From the HOV lane from North- or Southbound I-405:

  • From the carpool (far left) lane, take the exit onto NE 128th St.

  • At the top of the ramp turn left if Northbound, right if Southbound onto

  • NE 128th St. Go through the next light at 116th Ave NE.

  • At the second driveway, turn left into the parking lot at 11415 NE 128th St.

Park wherever you wish, and enter the  building from the upper parking lot.

When inside:
Evergreen Health and Wellness Offices, LLC, Suite 100, is immediately on your right as you enter the lobby. Enter the waiting room and press the signal next to my name to indicate your arrival. I will come to meet you at your scheduled appointment time. To find the restrooms, go back through the lobby and down the hallway to the left of the stairs.
